She has been complaining of dry cough since the past two weeks and low grade fever
that started two days ago, and was as high as 101 orally. She has had a decreased
appetite but no nausea and vomiting. The cough occurs during the night and she needs
to sit up in a chair to be able to breathe easier. The cough is mainly dry, rarely
productive.
She had been prescribed inhalers in the past; they have been helpful but she does not
use them on a routine basis. She has been prescribed antibiotics in the past as well and
that seems to help when she is acutely ill. She has been suffering from shortness of
breath for the past two weeks following any kind of activity mainly because of the dry
cough. She thinks it’s possible that there’s some problem with her “heart.” She is also
complaining of slight sore throat, especially in the morning and feels she may have lung
cancer.
The patient’s symptoms have been worsening over the past two days.
She has had similar episodes in the past. The last was three months ago when she had
to go to the emergency room and they told her that she needed to be hospitalized. She
declined hospitalization at that time and was treated and released. She says they gave
her antibiotics and an inhaler before discharging her. She mentioned that though it took
some time to feel better, there was gradual improvement in her condition following that
treatment. According to her, this is the worst episode that she can remember. She’s very
concerned today that she could have pneumonia and might require hospitalization.
She is seeking medical attention today because of the fever and prolonged nature of her

Introduction
Today more than 1.2 million people are living with the Human Immunodeficiency Virus (HIV) in the United States (CDC, 2016). The once terminal illness, which is the precursor to Acquired Immunodeficiency Syndrome (AIDS), has downgraded to a manageable chronic disease due to the creation of antiretroviral therapy (ART), improved health awareness, and behavioral prevention strategies.
Even though, various preventive interventions have been designed in efforts to prevent and decrease the incidence of HIV infections some populations are at increased vulnerability for contracting HIV. Historically, lesbians, gays, bisexual and transgender individuals (LGBT) have been identified as a vulnerable population due to prejudices against this group, discriminatory practices, and social stigma. The vulnerability of these groups become even more disadvantageous when health barriers such as reduced access to healthcare, substance abuse, mental health disorders and lack of health insurance become a factor in utilizing health care. These social determinants coupled with increased health disparities disproportionately places this group at a higher risk of HIV transmission.
According to the Center for Disease and Control (2016), men who have men who have sex with men represent 2% of the United States population but contribute to over 50% of people living with HIV (PLWH). It is estimated that one in six MSM will be diagnosed with HIV in their lifetime based on the current HIV infection rates (CDC, 2016). Transgendered persons are even more susceptible to HIV infection; they accounted for the highest percentage of newly diagnosed cases in 2013 (CDC, 2016).
Presently, culturally sensitive and competent directed education regarding LGBT persons has been explored as a solution to prevent and decrease HIV transmission rates. The implementation of a culturally and linguistically competent LGBT education and training has the potential to reduce stigma, decrease health disparities and increase social awareness

Just because information is easy to count or describe does not mean that it provides insight. For example, a health promotion program that seeks to teach adults in a community to eat fewer fatty foods in order to avoid heart disease. A report about how many heart attacks occurred in the community might provide some insight on overall cardiac health, but there is no way to collect data on the number of heart attacks that did not happen as a result of the program. Moreover, the number of heart attacks alone will not be a relevant data point without being linked to fatty food consumption. However, it may be helpful to look at trends in eating habits as well as trends in heart disease and heart attacks over a 10-year period.

As a public health professional, you may be asked to determine what needs to be evaluated (what is being measured) and why, and what data will yield relevant information. For this week’s Discussion, you will practice planning data collection for effective evaluation using a scenario before collecting data for your own programs.
