CHIP Program

The Community Health Improvement Partners (CHIP) is a voluntary collaboration of San Diego, California healthcare stakeholders including hospitals, community clinics, insurers, physicians, universities, community benefit organizations, and the County of San Diego. The members of CHIP are dedicated to the common vision of “achieving optimal health for San Diego County’s communities through collaboration and assessment.” Formed in 1995, the CHIP model is a community health improvement model to measure performance and drive policy development. This assignment examines the outputs of the CHIP model in the form of reports on the current state of the county.
