GROUP PROJECT: Mission of Mercy

You will be planning a summer mobile health clinic project for an underserved cluster of counties in (you determine the part of the country). The project is being funded by a wealthy physician who is a close colleague of your parents who wants to “give back.”

CAVEATS

I have provided a list of “caveats” to make your Mercy Mission more interesting. Each team should thoroughly address at least three caveats in your project. You must clearly discuss applicable course materials including laws, moral codes, ethics, applicable team based approaches, decision theories, and potential impact on patient outcomes. Clearly describe the “steps” or methodology your team used to resolve any problems or issues.

A few caveats.

One. The funder has an overindulged 22-year-old son who has floundered in and out of college since high school.He thinks it would be a great experience for his son and makes his presence on the mission a requirement. He must also be a part of the planning team and carry his own weight of the work.You have discovered that the son is a womanizer and is likely to try to hit on every female within ten feet.You have also discovered that he likes to party and drink and sleep it off the next morning. The group must devise a way to incorporate this unwilling participant without offending his father and totally disrupting the mission.

Two. A newly licensed general practitioner has agreed to join the team.The team is ecstatic until he is suspected of illegal drug use.No one has been able to prove it, but subtle signs are there.He performs well with patients, but can be withdrawn, moody and short-tempered with staff. His behavior is impacting the team’s ability to provide quality care. What strategies will be implemented to address his impact on the team?

Finally, one of the female members of the team is secretly attracted to the doctor. You are not sure you can trust her to support the team’s decisions and strategies to address the tension within the group.The team is concerned she may undermine their efforts.

Debbie is a devout Jehovah’s Witness and refuses a blood transfusion.She has come to the mobile clinic for prenatal care as she has not had any for seven months.More than likely, she will deliver while the Mission is serving her small community. Debbie is pregnant and has history of a bleeding disorder, and insists that if she hemorrhages during her delivery, the physician cannot give her any transfusions. What next? Does Debbie have the “right” to make the decision for her baby? How should the team approach Debbie?

Harry, age 65, has chronic asthma. Even though he is very allergic to cat fur, he is very attached to his two cats, Jon and Jovi. Harry does ok with is asthma and knows that smoking a pack of cigarettes a day does not help his situation. The team physician has treated Harry’s asthma and tried to counsel him and convince Harry to quit smoking. He/she is reluctant to prescribe Harry most asthma medication because of the adverse side effects, and the simple fact that a change in Harry’s lifestyle will do more good than any medication.While covering the case in a team meeting, the doctor blurts out, I’m sick of treating these ignorant people who refuse to help themselves. What is the team’s response? What are the ethical issues?

A patient visiting the Mercy Mission Mobile Clinic is a 32 year old female. She reveals to you that she has recently learned that she is HIV positive. She is very candid with you and provided a comprehensive list of all the medications she is currently taking. She is rather nonchalant about her condition and even states that she was probably a bit too wild in her younger days. Approximately 3 weeks later, your sponsor’s son brings her to a staff cookout.
